(1)标题
有两种写法，一是直接写“请求函”，另一是在“请求函”前加上内容，如“协助请求函”一般采用第二种;
(2)称谓
顶格写明接受请求书的单位、组织或有关领导。
(3)正文
正文部分是请求书的主体，首先提出请求，其次说明理由。理由要写得客观、充分，事项要写得清楚、简洁。
(4)结尾
写明惯用语“特此申请”、“恳请领导帮助解决”、“希望领导研究批准”等，也可用“此致”“敬礼”礼貌用语。
(5)署名、日期
个人请求要写清申请者姓名，单位请求写明单位名称并加盖公章，注明日期。
